Please note that before you can begin working for Ski Ward, Inc. you will need to complete following:

• If you are 14 or 15 years of age you must acquire an Employment Permit from the Superintendent of Schools in the city or town you reside in the following manner:

• Get “Promise of Employment” card from your school Guidance Department or School Superintendent.

• Take Promise of Employment to Ski Ward where we will sign and return form to you.

• Take Promise of Employment to person giving physical exam.

• Take signed Promise of Employment back to Guidance Dept. with Proof of Age (birth certificate, passport, baptismal certificate, immigration record, or religious or official record bearing minor’s age).

• Return completed Employment Permit to Ski Ward, Inc.

• If you are 16 or 17 years of age you must acquire an Educational Certificate from the Superintendent of Schools in the town where they reside or are employed, in the following manner:

• Take Proof of Age (birth certificate, passport, baptismal certificate, immigration record, or religious or official record bearing minor’s age) to School Superintendent or Guidance Department.

• Return completed Educational Certificate to Ski Ward, Inc.

• Complete, sign and return W4 form.

• All Employees must complete Employment Eligibility Verification (I-9) in the following manner:

• Complete and sign Section 1 of I-9 form.

• Bring original or copies of documents from List of Acceptable Documents on back of I9 to Ski Ward, Inc.

• Ski Ward will verify identification and sign I9. Your I-9 is not complete until it has been signed by authorized representative of Ski Ward, Inc.
